Working as a home health aide with mentally ill patients requires a special sensitivity to the needs of the mentally challenged. Too often, people tend to think of the mentally challenged as being somehow inferior and as such, they tend to talk down to them. However, what the mentally ill really need is what anyone else needs – they need someone who will take them seriously and make sure their needs are met while giving them as much freedom as possible.

Just What Is Mental Illness?

Before we can begin to discuss how to work effectively with mentally ill home care patients, it's important to understand what mental illness actually means. Let's start with the most basic definition and then go into a more detailed explanation. At its most basic, mental illness means that someone is incapable of functioning independently in society due to some kind of problems with how they experience the world emotionally. Exactly how this manifests itself and what the issues are do require additional explanation however.

Some indicators that a person may be mentally challenged include:

  • Inability to Develop and Maintain Healthy Relationships – Many mentally ill people find it difficult or impossible to create healthy interpersonal relationships.
  • Lack of Impulse Control – Another common problem for mentally challenged individuals is a lack of impulse control. Many mentally challenged individuals will for example do things in public or even in private that mentally sound people will not do (ie scream out loud at no one in particular, cause damage, public urination, etc.).
  • Inability to Tolerate Anxiety and Frustration – We all deal with anxiety and frustration on a daily basis. However, those who are mentally challenged often find that they can not handle any (or very little) anxiety and or frustration in daily life.
  • Inability to Respect Others – Finally, many mentally ill people find it difficult to respect others, in terms of private space and or in terms of regular verbal abuse.

It is important to realize that the above list includes things that even mentally healthy individuals experience from time to time. The key difference is that mentally challenged individuals will experience these things on a regular basis and will lack the impulse control to display normal behavior most of the time.

Cause of Mental Illness

Mental illness is a disease and typically a chronic disease just like diabetes, arthritis or RLS. In other words, mental illness is not the fault of the person experiencing it and the mentally challenged should not be made to feel bad for their disability. Some common causes of mental illness include:

  • Physical Factors – Trauma to the head, either in childhood or adulthood can cause permanent mental illness to occur.
  • Chemical Imbalance – Many mentally ill individuals have a chemical imbalance in the brain which can sometimes be corrected through the use of medication.
  • Environmental Factors – Finally, mental illness can be either temporarily or permanently caused by environmental factors. For example, children who are exposed to severe trauma often grow up to be mentally challenged individuals who may experience lifelong problems. Victims of terror attacks or violent crime also sometimes develop either temporary or permanent mental illnesses as a result of their experiences.

Types of Mental Illness

Mental illness typically takes on a number of different forms. These include:

  • Anxiety – We all experience a certain amount of anxiety in our daily lives. However, those who are mentally challenged may experience severe anxiety which is disproportionate to the problems they happen to be experiencing.
  • Fears – Similar to anxiety, fears in the mentally ill will include severe, crippling phobias which are not found amongst ordinary people. For example, an ordinary person may be repulsed by the idea of ​​being near an open sewer. However, the individual will have such paranoia from being near the open sewer that he may refuse to leave his home for fear of the possibility of passing by an open sewer.
  • Depression – Again, while we all feel a normal amount of depression from time to time, someone who is mentally challenged will experience severe depression, often for no reason other than being depressed.
  • Paranoia – Finally, mentally challenged individuals may experience severe paranoia to the point where they are unable to function in society.

Thus, they may need help only on a temporary basis rather than on a permanent basis.

Your Responsibilities

As a home health care worker with people facing mental challenges, there are a number of things that you need to do in order to ensure the safety and development of your charges. These include:

Record Keeping

You will need to keep careful track of your patient's emotional states. This means observing changes in behavior and noting what may have precipitated them. You should also note whether or not the change lasts for a long period of time or if it is a short lived change.

Your mentally challenged patients may also experience changes in behavior which are of a positive nature. Thus, it's important to note when and if such changes occur and to note changes in personality, either for the good or the bad.

While you should note environmental factors surrounding such changes in behavior, it's important not to draw conclusions on your own as to what precipitated these changes. Instead, you should note only the facts and allow mental health professionals to make such diagnoses.

Making Sure Patients are Cared For

It's important as a home health care worker working with mentally ill individuals to make sure that they are complying with their treatment regimen. This means for example that you need to ensure that the person is taking their medication on time and in the correct doses.

You must also watch carefully to ensure that dangerous behaviors do not put your patients in harm's way. When danger does present itself, you should call 911 to get emergency workers to help deal with the problems rather than attempting to deal with it on your own. For example, if your mentally challenged patient overdoses on sleeping pills, you should call 911 immediately rather than try to induce vomiting on your own.

Provide Patients with a Path to Recovery

Finally, your role as a home health care worker is to facilitate the patient's path to recovery. This means that you work with them to ensure that they will have the right kind of support to deal with their problems and that you help to keep the family involved as well in the recovery of the mentally challenged patient that you work with as a home health care worker.

Eliminating that Terrible Hum From Your Home Theater Speakers

You’ve all heard it, that dreaded 60Hz hum through the speakers of a home theater or house audio system. Hopefully you heard it at a friend’s house and not your own. It can drive you completely nuts. You may have even tried, unsuccessfully, to fix the little noise problem. That can make you even more crazy. What causes that horrendous noise through your speakers?

More often than not humming through your speakers is caused by a grounding problem. There are three main ground problems that cause problems in an audio / video system. These are ground loops, improper grounding and lack of a ground altogether. The other possible culprits that can cause noise are bad cables, a faulty piece of equipment or electrical noise from a lighting dimmer or electric motor. There are steps you can take to troubleshoot the noise and eliminate it from you theater.

The first step is find out where it is coming from. Disconnect your source and display equipment from your receiver or surround sound processor. If the noise stops, connect them back to the receiver or processor on at a time until the noise returns. When the hum comes back, you found where the noise is entering your system. Note that if you are connecting remote equipment, such as running the signal from your theater room DVD player to the TV in the bedroom, your chances to pick up noise increase dramatically. With such long runs, noise can be induced into the long cable runs from adjacent electrical wiring. It is also easy to create a ground loop, because the equipment is plugged into two different, widely separated outlets, on different electrical circuits.

If the noise is caused by a cable box, the noise is likely caused by the cable TV ground. To test this theory, disconnect the incoming cable TV feed to the rear of the cable box or TV while they are still connected to the rest of the system. If the noise is eliminated by disconnecting the TV cable, the problem is the cable TV ground. You can electrically decouple the cable TV feed from your system with a ground breaking transformer. These are available from many sources. Be advised that many newer, digital cable TV systems require any device in the signal chain to pass a full 1,000 Mhz. Some of the older ground break transformers will not do this. Be sure to check the specifications of whatever device you are purchasing to verify it will pass the digital cable TV signal.

If the noise is from your projector, TV, or monitor, it is most likely caused because the video display device is plugged into a different outlet than the other a/v equipment. It could be on a different circuit as well. These circuits may have two different ground potentials. That is, the resistance to ground is different on each circuit. A difference in resistance to ground from one ground point to another can cause the dreaded ground loop. If you get a ground loop, current flows between the two components. If the current flows through the components internal audio signal ground, you will get a hum.

You can use an isolation transformer, similar to the type used for cable TV ground problems, to eliminate the electrical connection from one component to the other. These transformers are inserted in line with the audio signal connection between the two components. If there is no audio connection between the components, the problem may be current flowing through the video portion. In this case, a video isolation transformer should be used to eliminate the ground loop.

Sometimes power conditioners will stop noise problems by placing equipment on different, electrically isolated outlets. This is done using isolation transformers. Sometimes this is ineffective however, due to the differences in internal construction of different power conditioning equipment. Some safety regulations, such as UL 1950, specify that an isolation transformer is only allowed to isolate the hot and neutral wires; the grounding wire must be passed straight through. If this is the case, the ground loop problem may still exist because many communication circuits are connected to the grounding conductor and not the neutral. In this case, the isolation transformer, or any power conditioner or UPS with an isolation transformer will have absolutely no affect on the grounding problem.

The noise may be generated externally, from a dimmer or refrigerator compressor for example, and coming in through the main power input on the audio video equipment. In this case, a high quality power conditioner may be effective in reducing or eliminating the noise problem. You may also find that one of the signal interconnecting cables in your system is faulty. This can also cause noise problems. Check for this by swapping the cables with one that you know to be good.

You can solve most noise problems in your home theater or multi room audio/video system by taking the systematic, step-by-step approach. Work your way up the signal chain, eliminating each piece of equipment as you go. If you have nothing connected to your speakers except the speaker wiring, and they still hum, the problem is noise induced into the speaker wiring from adjacent power cables. Other than that case, most problems are caused by ground problems, which you can find, and solve, if you take it one step at a time.

The Three Basic Types of Home Foundations

There are three basic types of foundations in use today in the construction of a home.

Slab Foundations

A slab foundation is a foundation built directly on the soil with no basement or crawl space. Slab foundations are common in areas where soil conditions are not suitable for a basement, and are the most common foundation found in warmer areas such as Florida, Arizona, California and Texas, or anywhere where the depth between the soil and stable underlying rock is very shallow. Slabs are the quickest and cheapest foundation because they require less labor, skill and materials cost. They consist of a concrete slab that is typically 6 to 8 inches thick. Embedded within the slab is a grid of supporting ribbed metal rods known as “rebar.” Even in locations where basements are prevalent, slab foundations are typically laid to serve as the base for structures like garages, pole barns, and sheds. Slabs are the least expensive of the three main foundation types but provide no storage or utility space, as the home actually sits directly on a large platform of solid concrete. Slabs have the disadvantage of being difficult and expensive to repair when they settle and crack, and plumbing lines that protrude from the soil upward through the concrete can also be expensive to repair. In areas where the underlying soil is thick or prone to excessive expansion and contraction, cables are embedded which can be tightened to provide better horizontal support and minimize the width of cracks.

Crawl Space (Pier and Beam) Foundations

A pier and beam foundation consists of either vertical wood or concrete columns (piers) that support beams or floor joists above the ground. The areas between the soil and the bottom of the house floor is known as the crawl space. These foundations are built either at ground level or over a shallow excavation that varies in depth, but is commonly about 36 to 40 inches deep. The best crawl space foundations have a load-bearing concrete perimeter wall and concrete or steel piers, both having footings below the freeze line of the soil, along with a good barrier over the soil to keep moisture under control. Less expensive versions have no load-bearing perimeter walls, piers with shallow footings, and no moisture barrier at all over the soil. Crawl spaces that enclosed by a wall or by skirting must have vents on every side to allow air to circulate and help keep the soil dry under the home. These vents must be configured to prevent the entry of rodents and snakes. Crawl space foundations are most often used in areas where there is heavy clay content in the soil that can severely damage (crack) slab foundations, or in waterfront or flood prone building sites where the necessary floor height to prevent water penetration of the living space must be higher than a slab can normally provide. The primary advantages of crawl space foundations are that plumbing lines are readily accessible for repairs, and foundation settlement problems are easier and less expensive to correct than with slab foundations. A primary disadvantage occurs when these foundations are not properly maintained or are constructed without adequate ventilation, allowing water or pests to cause damage. Crawl space foundations without adequate insulation applied to the bottom of the house floor can be very energy inefficient in a cold climate.

Basement Foundations

A basement is a type of foundation which includes an accessible space between the soil and the bottom of the first floor of a home. This foundation provides living space below the home, below the ground elevation. It is basically a slab foundation with walls and a floor. Basements are most often built in cold weather climates such as the Northeast, Midwest and Rocky Mountains, and in places where the cost of excavation is not prohibitive. Basements start with a hole approximately 8 feet deep, however, some homeowners will opt for a 9 or 10 foot deep basement wall to increase height and volume of useable space. The floor and walls are built, then the house itself is built over that. Basement foundations have the advantage of providing useful space for utilities, mechanicals systems, and storage not available in the previous two types of foundations. The primary disadvantage of basements is that because they are mostly below ground level, they are vulnerable to leakage, mold formation, and flooding. Basements in wet climates must always have a working drain and pump in the floor to combat flooding.
